What to Bring

• Driver’s License or Valid Photo ID
• Insurance Card
• Medical Records (X-rays, MRI, previous treatment reports, any other relevant information)
• A list of current medications (if any)

On the Day of Your Appointment

• Arrive 15 minutes early to complete any additional paperwork.
• Wear comfortable clothing that allows easy access to the area being evaluated.

Insurance & Payment

We accept most major insurance plans. Please contact our office to verify your coverage. Co-pays and payments are due at the time of service.

Cancellation Policy

If you need to reschedule or cancel your appointment, please provide at least 24 hours’ notice.
